About this session

Medication support can range from occasional reminders to helping an older relative manage several prescriptions, inhalers, creams, over-the-counter products and supplements.

Changes in memory, vision, hearing, dexterity, swallowing, mobility or daily routine may make a previously manageable medication plan more difficult.

This session helps participants provide useful support without automatically taking over.

They learn how to build an accurate medication record, identify where the routine is breaking down, prepare for appointments and discuss practical concerns with pharmacists, doctors and other qualified professionals.

The emphasis is person-centered support. Wherever possible, the older relative remains involved in decisions, understands what is being discussed and retains control over the parts of medication management they can safely handle.

Structured medication reviews are designed to involve the person in decisions and can help reduce harm from side effects, interactions or an unmanageable treatment burden, as NHS England guidance on structured medication reviews notes.
