About this session
A load falls but misses a worker. A forklift and pedestrian stop just in time.
A machine unexpectedly moves while someone is nearby. A spill is discovered before anyone slips.
No injury occurred, but the event may reveal the same conditions that could produce a serious incident next time.
Participants learn to recognize near misses, hazardous conditions and other weak signals without turning reporting into a hunt for trivial mistakes.
They focus on what happened, what could credibly have happened, which controls were missing or failed and what information the next person needs in order to act.
The session also develops a proportionate response. Not every report requires a full investigation, but some near misses have serious potential even when the actual outcome was minor.
Participants learn how organizations can triage reports, identify high-potential events, combine similar signals and escalate the issues that deserve deeper review.
Longer programs move from individual reporting into organizational learning. Participants examine reporting barriers, recurring themes, corrective-action follow-through and the feedback employees receive after raising concerns.
The goal is not to maximize report counts. It is to create a reliable flow from signal → review → action → learning → prevention.
What your people take away
- Recognize near misses, hazardous conditions and weak safety signals worth reporting
- Describe events factually without blame, diagnosis or unsupported causal assumptions
- Distinguish actual outcome from credible potential severity
- Triage reports so higher-potential events receive the appropriate level of review
- Identify recurring patterns across tasks, locations, equipment and work conditions
- Close the reporting loop through action, feedback and shared prevention learning

What each length covers
30 minutes
30 Minutes — Key Points: Recognize the Signal Before Someone Is HarmedA focused 30-minute session on three key points from Recognize the Signal Before Someone Is Harmed: understand what a near miss can reveal, distinguish events, conditions and observations and look at potential — not just the actual outcome.
- Understand what a near miss can reveal
- Distinguish events, conditions and observations
- Look at potential — not just the actual outcome
Covers 3 of 4 points in Recognize the Signal Before Someone Is Harmed.
60 minutes
60 Minutes — Near-Miss Reporting EssentialsA focused introduction to what near misses reveal, how to report them clearly and why actual outcome alone can hide serious potential. Participants improve a weak report and decide which signals require immediate escalation.
- Near miss versus hazardous condition
- Actual outcome versus potential severity
- Immediate safety action
- Factual reporting
- Observation versus assumption
- Useful reporting detail
- High-potential near misses
- Near-Miss Reporting Exercise
- Live Q&A
Covers 1 of 4 parts: Recognize the Signal Before Someone Is Harmed.
90 minutes
90 Minutes — Report, Triage and LearnThe extended session develops reporting quality, high-potential-event recognition, proportionate triage and recurring-pattern analysis. Participants work through realistic reports and connect weak safety signals with stronger preventive controls.
- Near-miss recognition
- High-potential events
- Reporting quality
- Context without blame
- Triage and escalation
- Proportionate review
- Recurring patterns
- Control improvement

What’s covered
Recognize the Signal Before Someone Is Harmed
Every length- Understand what a near miss can revealA near miss is an event in which harm did not occur but the circumstances could have produced injury, illness, damage or another serious outcome.
- Distinguish events, conditions and observationsA near miss involves something happening; a hazardous condition may be discovered before an event occurs.
- Look at potential — not just the actual outcomeTwo events can both end without injury while having very different potential consequences.
- Act on immediate danger firstReporting does not replace immediate safety action.
Report What Happened Clearly and Factually
90 minutes and longer- Describe the event — not the personUseful reports describe the task, location, sequence, equipment, conditions and actions observed.
- Capture the conditions around the eventTime pressure, equipment condition, visibility, staffing, maintenance, congestion, weather, workload and simultaneous activities may all provide useful context.
- Separate observation from assumptionParticipants distinguish what they directly observed, what another person told them and what remains unknown. 'The barrier was lying on the floor when I arrived' is different from 'someone removed the barrier because they were rushing.'
- Make the report easy to act onLocation, task, time, equipment, photographs where appropriate, immediate action and a concise description of what could have happened can make a report more useful.
Triage, Learn and Find the Pattern
3 hours and longer- Identify high-potential near missesSome events deserve rapid escalation because a small difference in timing, position or circumstances could have produced severe harm.
- Match the review to the signalA simple housekeeping concern may be corrected and tracked without a formal investigation, while a near miss involving uncontrolled energy, vehicle interaction, a dropped load or another serious hazard may justify deeper review.
- Look for recurrence across reportsOne blocked aisle may be a local issue; repeated blocked aisles across shifts may indicate storage capacity, workflow or housekeeping-design problems.
- Ask which control needs to changeNear-miss learning should lead back to prevention.
Close the Loop and Build a Learning System
Full day only- Show people that reporting leads to actionEmployees are less likely to keep reporting if concerns disappear into a system without visible follow-through.
- Remove the barriers to speaking upComplex forms, lack of time, unclear reporting routes, fear of blame and previous inaction can all reduce reporting.
- Use reporting data without rewarding silenceA rise in near-miss reports can reflect greater exposure, better reporting confidence or both.
- Share learning beyond the original eventA control weakness found in one location may exist anywhere the same equipment, process or work pattern is used.
