A sample prevention programFour offices, three cost drivers, three years
This is what the free Prevention Impact Planner makes, word for word: first the plan, made in November 2026, then the 12-month check in December 2027. Corvane is a fictional company, and every number is invented. Yours will follow your sites, your data and your budget.
Corvane is a fictional company, and every number here is invented. This sample shows what the Prevention Impact Planner makes: first the plan (made in November 2026), then the 12-month check (December 2027).
Corvane makes payroll and payments software. It has 6,000 people in four offices: Toronto (headquarters, 1,400 people), Warsaw (finance and operations, 1,300), Bengaluru (technology, 1,800) and Manila (customer support, 1,500, on shifts around the clock). A three-person wellbeing team in Toronto runs the program for all of them.
Part 1: The plan (November 2026)
Step 1: The cost drivers
All four offices share the same top three, so this is one table, with the sites that stand out. Corvane's group health plans are insured in all four countries, so claims drive the renewals: the insurers paid about US$7.2 million in claims in 2026.
| Rank | Cost driver | Evidence from your data | Yearly cost (estimate) | Prevention can influence it | Why it's a priority |
|---|---|---|---|---|---|
| 1 | Heart and circulation | 18% of claims; 31% with raised blood pressure in health checks (Warsaw 34%, Manila 33%); 34% say they know their blood pressure | US$1.3 million in claims | High | The largest share of claims, common and often silent, and it responds to checks and follow-up |
| 2 | Cancer | 14% of claims; 52% of eligible employees up to date with screening (insurers' reports for Toronto and Warsaw); 17% tobacco use in health checks | US$1.0 million in claims | High: early awareness and risk factors | Large claims, and screening has room to grow |
| 3 | Musculoskeletal (back, neck and joints) | 12% of claims; 21% of absence days, and the top cause of absence in Manila and Warsaw | US$0.9 million in claims, plus about US$0.6 million in pay for absence days | High | Two costs at once, and quick changes to workstations and shifts help |
Data used: health check group summaries from the clinic provider (all sites, 3,900 people took part), insurers' claims reports by category, absence by cause and turnover from the HR system, and the pulse survey. Data to collect: screening uptake from the Bengaluru and Manila insurers.
Step 2: The baseline (December 2026)
| Driver | Measure | Type | Year-zero value | Source |
|---|---|---|---|---|
| Heart | Raised blood pressure in health checks | Leading | 31% | Clinic group summary |
| Heart | "I know my blood pressure" (agree) | Leading | 34% | Pulse survey |
| Heart | Heart and circulation share of claims | Lagging | 18% | Insurers' reports |
| Cancer | Eligible employees up to date with screening | Leading | 52% (Toronto and Warsaw) | Insurers' reports |
| Cancer | Tobacco use in health checks | Leading | 17% | Clinic group summary |
| Cancer | Cancer share of claims | Lagging | 14% | Insurers' reports |
| Musculoskeletal | Strength or movement exercise at least twice a week (agree) | Leading | 22% | Pulse survey |
| Musculoskeletal | Absence days per employee for this cause | Lagging | 1.1 | HR absence records |
| All | Absence days per employee, all causes | Lagging | 5.2 (Toronto 4.4, Warsaw 5.6, Bengaluru 4.2, Manila 6.8) | HR absence records |
| All | Voluntary turnover | Lagging | Toronto 12%, Warsaw 15%, Bengaluru 19%, Manila 34% | HR system |
VOI scorecard, all sites: reach (the share who join at least one activity), know your numbers, manager support, intent to stay and energy. The same five pulse questions every December, in every office's languages.
Step 3: The plan
Year 1 (2027): three threads, repeated all year
| Driver | The message | Surges and second moments | Share of Year 1 budget |
|---|---|---|---|
| Heart and circulation | "Know your numbers, and act on them." | Main surge: February health check week (Heart Month). Second: World Heart Day, September 29 | 41% |
| Cancer | "Know the checks for your age, and book them." | Main surge: the same February week (World Cancer Day, February 4). Second: men's health in November | 31% |
| Musculoskeletal | "Strengthen and move every week, and speak up early." | First touch: April, personal exercise plans. Main surge: October (Global Ergonomics Month, World Spine Day) | 24% |
The other 4% covers January's groundwork and December's check.
| Month | Thread and cycle stage | Moment | Live touch: Workbliss session, format, sites | Evidence: intervention and source | Message (all sites, local languages) | Manager or champion action | Budget share |
|---|---|---|---|---|---|---|---|
| January | All three: groundwork | First week back, January 4 to 8 | Understanding Your Health & Healthcare (online, all sites, at two times) | Health education, part of the combined approach below (CPSTF) | "This year, know your numbers. Health check week is in February: book your slot." | Share the three messages at the first team meeting of the year | 2% |

What's one thing you'll do?" | 6% | |
| April | Musculoskeletal: awareness, first touch | Musculoskeletal Health Clinic (in person, Manila and Warsaw): a physiotherapist's check and a personal exercise plan | Exercise plus education (Steffens et al., JAMA Internal Medicine, 2016) | "Strong backs are built: ten minutes, three times a week." | Start one team meeting a week with a two-minute stretch | 7% | |
| May | Heart: skills | World Hypertension Day, Monday, May 17; World No Tobacco Day, Monday, May 31 | The Everyday Heart Health Series begins (online, weekly, at two times) | Health education program (CPSTF, strong evidence for blood pressure and cholesterol) | "Small swaps: salt, steps, sleep." | July | Musculoskeletal: skills | Resistance Band Pilates, weekly for eight weeks (online, with on-shift times in Manila) | Exercise (Steffens et al., 2016) | "Movement breaks that fit a shift." | A two-minute stretch at shift start in Manila, and stand-up meetings elsewhere | 5% | |
| August | Heart and cancer: opportunity | No live session. Paid time off for screening appointments is approved and announced | Reducing structural barriers to screening (CPSTF, strong evidence) | "Your screening time is paid. Book it." | Approve screening time without fuss | 0% | |
| September | Heart: awareness again, the second cycle | World Heart Day, Wednesday, September 29 | Workplace Health Check and Know Your Numbers Clinic again in Warsaw and Manila, the sites with the most raised readings | Health check with feedback (CPSTF); people with raised readings encouraged to measure at home with their doctor's support (CPSTF, strong evidence) | "Check again: how have your numbers moved?" | Remind the team, and share the company-wide sign-ups | 10% |
| October | Musculoskeletal: the main surge | Global Ergonomics Month; World Spine Day, Saturday, October 16 (marked Friday, October 15) | Resistance Band Pilates in person at every office; Musculoskeletal Health Clinic again in Manila and Warsaw; Workstation Assessment: Computer Display Safety (online), for comfort | Exercise plus education (Steffens et al., 2016). Workstation setup supports comfort, but doesn't prevent back pain on its own | "Speak up early: discomfort is easier to fix this week than next month." | Book the team into a session | 12% |
| November | Cancer and heart: awareness again, the second cycle | Movember; International Men's Day, Friday, November 19 | Men's Health Screening & Consultation Clinic and Women's Health Consultation Clinic (in person, all sites); Heart Health for Men Program (online) | One-to-one education for screening (CPSTF); heart health education (CPSTF) | "The checks for your age, booked this month." | Book group slots for the team | 10% |
| December | All three: the 12-month check | No live session. The pulse survey, plus group summaries from the clinic provider and insurers | "What we did together this year." | Thank the team, and share the company-wide results | 2% |
- Every message and reminder counts too: for screening, reminders and small media (posters, emails, short videos) are both recommended on strong evidence (CPSTF). Together, these activities make a multicomponent program, which is also recommended on strong evidence.
- Frontline and shift teams: every live touch in Manila has an on-shift time, and every message also goes on the break-room screens.
- Languages: English in Toronto and Bengaluru, Polish in Warsaw, and English and Tagalog in Manila.
- Measured in December 2027: every leading measure, absence, turnover and the VOI scorecard. Claims are shown but not counted yet.
Year 2 (2028): go deeper where risk is highest
| Quarter | Site | Driver | Activity and Workbliss sessions | Format and language | Who it's for | Measure it should move |
|---|---|---|---|---|---|---|
| Q1 | All | Heart and cancer | February health check week again, with one-to-one follow-up for raised readings: Workplace Health Screening & Risk Assessment Clinic | In person, local languages | Everyone; one-to-ones for anyone who chooses | Raised blood pressure; screening uptake |
| Q2 | Warsaw and Manila | Cancer risk factors | A smoke-free workplace policy check, quit support with incentives, and Movement for Metabolic Health | Online and in person | Everyone | Tobacco use and risk factors in health checks |
| Q3 | Manila | Musculoskeletal | Resistance Band Pilates built into shift schedules; Occupational Health Consultation Clinic for people with ongoing pain | In person, on shift | Support agents and team leads | Absence for this cause |
| Q4 | All | Heart | The Everyday Heart Health Series, second run; Heart Health for Men Program | Online | Everyone | Raised blood pressure |
Year 3 (2029): make it last. Peer champions in every office, the three messages in onboarding and manager routines, the health checks repeated, and the full three-year comparison.
Step 4: The comparison
- Absence and turnover: each office is compared with its own 2024 to 2026 trend. Warsaw's absence was rising by about 0.3 days a year, and Manila's by about 0.2.
- Claims: compared with each insurer's projected trend for the group.
- Limits: a trend can change for reasons outside the program, and claims in a group this size can swing with a few large claims, so they're averaged across years.
Next 30 days
- Book February's health check week with the clinic provider for all four offices, with on-shift times in Manila.
- Brief managers on the three messages and their two-minute actions.
- Ask the Bengaluru and Manila insurers for screening uptake, so cancer screening can be tracked at all four offices.
Data still to collect: screening uptake from the Bengaluru and Manila insurers, and the five pulse questions added to December's survey (HR).

Step 6: The impact report
Headline: in its first year, the program returned an estimated 1.1 times its cost (range 0.6 to 1.7), mainly through lower absence in Warsaw and Toronto and lower turnover in Toronto and Bengaluru. Claims aren't counted yet.
The calculator's output (exchange rates are examples):
| Site | Measure | Change | Result | Value | Counted in ROI |

| Manila | Turnover | 34.0% to 32.5%; comparison -0.5 pts; net -1.0 pts | 15.0 leavers avoided | USD 53,550 | Yes |
| Case | Share credited to the program | Adjusted benefit | Benefit-cost ratio | ROI |
|---|---|---|---|---|
| Low | 25% | USD 133,566 | 0.56 | -44% |
| Central | 50% | USD 267,132 | 1.11 | +11% |
| High | 75% | USD 400,698 | 1.67 | +67% |
Program cost for 2027: US$240,000, including delivery and staff time. Confidence adjustment: 80%.
VOI scorecard:
| Measure | Baseline | Year 1 |
|---|---|---|
| Reach: joined at least one activity | Not running | 68% |
| Know your numbers | 34% | 61% |
| "My manager supports my wellbeing" | 55% | 61% |
| "I see myself here in two years" | 58% | 62% |
| "I have the energy I need for my work" | 52% | 57% |
Assumptions: daily pay is salary divided by working days, without the cost of cover; replacement cost is half a year's salary, the low end of Gallup's estimate; 50% of the improvement is credited to the program in the central case; claims are shown but not counted in year 1.
What's next: 1. Add one-to-one follow-up for raised readings to February's health check week. 2. Ask the Bengaluru and Manila insurers for screening uptake, so cancer screening is tracked at all four sites. 3. Keep October's back-and-joints surge, and build short strength sessions into Manila's shift schedules.
